2023-2024 Athletics Registration is now OPEN through Final Forms. Every student-athlete must be registered in Final Forms in order to participate.

Parents/Guardians: Visit this link: www.portage-mi.finalforms.com to create an account for your individual student-athlete. Upon completing the initial questionnaire that involves general information, contact information, and health history, you will be done with this process for the rest of the time your student participates in sports at Portage Central. 

Students: Once your parent has created your account, make sure to activate it. You will receive an e-mail through your Portage Public Schools E-Mail. CHECK BACK OFTEN! 

Both Parents and Student-Athletes will now have the chance to sign critical compliance forms and receive timely e-mail communication from the Athletic Office and each individual program.
